Dear Sir/Madam,

With respect this is to notify you that your services with our organization have been terminated in accordance with our organization rules and regulations.

We were very pleased with your performance when we hired you but from some time your performance and behavior with other employees were found dissatisfactory. You were given three oral warnings and two written warnings about your action but we found no improvements from your side. We are very disappointed on your current performance and behavior. We want to keep you as valuable employees but we cannot keep you because management made its decision.

I hope we will create a professional relationship in future when you will improve yourself. For now I apologize on this matter, I cannot help you. I wish you for a bright future in your career.
